I want to share the amazon prime membership with 3 more person. All I need is name, email, birthday(month and day).

I start the trial membership today and will upgrade to the regular membership 1/15/2010. I'm not sure whether it will end 12/15/2010 or 1/15/2011. But you will get at lease 1 years amazon prime membership for sure.
