多谢MM，也祝你们申请一切顺利。 我们为了省心，还是找了律师，这是当时给的check-list。除了下面这些自己需要提供的证明，其它就是把所有申请的表格信息 (I-485, I-130, I131, I765, I884) 都填好。

Documents from Petitioner:

1. Copy of Naturalization Certificate, or U.S. passport, or U.S. birth certificate;

2. Copy of birth certificate;

3. Copy of parents’ marriage certificate if petitioning for father or vice versa;

4. Most recent federal income tax return including all schedules and W-2 Form; and

5. Letter from employer verifying current employment and salary, or three most recent pay stubs.

Documents from Applicant:

1. Birth certificate for all applicants. Birth certificates must include your complete name, parents' complete names, place of birth, and date of birth, and be registered within three months of the birth. Any birth certificate lacking all of the aforementioned information may also require secondary documentation (e.g. notarized affidavits and school leaving certificate)

2. Passport copies:

2.1. Please provide complete color copies (including ALL blank pages) of each applicant’s current passport. The copies should be color, scanned copies, and slightly enlarged. When making the copies, please move any attached I-94 cards, etc. out of the way.

2.2. Please also provide complete color copies (including ALL blank pages) of each applicant’s previous passports if they contain a U.S. entry stamp or U.S. visa stamp.

3. Copy of current Form I-94 for all applicants (obtain at [url=https://i94.cbp.dhs.gov]https://i94.cbp.dhs.gov[/url])

4. Copies of all previously issued Forms I-20, DS-2019, I-797 Approval Notice, and EAD cards (if applicable);

5. Six (6) color passport style photographs, 2x2 inches, for each applicant aged 14 and older; and four (4) for each child under 14 years of age; and

6. Completed medical examination for each applicant. Applicants are required to appear at a USCIS certified physician to complete the exam and the form.
