前資深運動主播傅達仁，這幾日頻頻在臉書上抒發感想，表示前往瑞士是「最後的旅途」，今（11/10）天更直言已成為國際人士安樂善終組織—「尊嚴」（DIGNITAS）的合法會員，讓不少人臆測傅達仁這趟是「安樂死」之旅。

84歲的傅達仁，因胰臟癌的折磨，讓他曾上書總統，一求安樂死能合法化，日前更是宣布他已加入瑞士安樂死組織「尊嚴」。讓不少人開始好奇這個組織，到底是怎麼運作的？又為何會吸引如此多國際人士加入會員？

瑞士的安樂死　其實是協助自殺

安樂死可分為

主動安樂死（Active Euthanasia），與被動安樂死（Passive Euthanasia），前者即為主動為病人結束生命，例如透過注射方式；而後者即為停止療程，例如除去病人的維生系統、或是讓病人停止服藥，讓病人自然死亡。 

而一般針對安樂死立法的討論，多聚焦於主動安樂死，以及醫師協助自殺（Assisted Suicide），即為由病人「自主」服下致命藥物。 

然而在瑞士，由他人透過注射而死亡的「主動安樂死」仍為非法，主張方法是醫師協助自殺（Assisted Suicide），由病人自主決定是否喝下、或是按下含有致命藥物的點滴開關。

想要安樂死　需先上繳會員費

尊嚴（DIGNITAS）於1998年為一位瑞士律師成立，是瑞士境內唯二協助安樂死的組織，因不同於另一組織出口（EXIT）僅收瑞士國民，因此世界各地有求死意願的人士，都會前往尊嚴（DIGNITAS）。

DIGNITAS採取會員制，欲入會的成員可在線上註冊、填寫資料，隨後DIGNITAS審核過後會寄發「入會註冊說明書」與「會員費帳單」。入會費加上年費約300美金，同時也須檢附上自己的醫療紀錄，並在另外繳交約4000美金，讓瑞士當地醫師檢閱是否符合資格。

為把關安樂死程序，成為DIGNITAS會員之後，仍須會診組織的診所護士，與另一位中立醫生。這位醫生要分別在面談之前和之後與病患各見面一次，負責評估病患所提供的資料。同時也會要求病患填寫求死證明書，並由兩位中立的見證人簽名。

若是該客戶因病無法親自簽名，會改採錄影的方式。影片中會確認客戶的身份，詢問他們是否求死，且是否出自自由意志而不受任何型式的強迫。

等一切溝通完成，醫師會再開出藥物，為病人安排人生最後一段的旅途，而上述的服務內容，需另外繳交約4000美元，同時事後火葬、死亡證明發放費用則是額外計算。

Advisory work / Counselling

Within the limits of the possibilities available to the association, DIGNITAS counsels its members and also non-members in regard to all end-of-life issues and in difficult circumstances of life.

Membership

DIGNITAS is a militant self-determination, autonomy and dignity help-to-live and right-to-die association, and fights internationally for “the last human right” on a legal and political level. Members have access to the services of DIGNITAS and show their solidarity with others who care for freedom of choice in life and at the end of life.

Suicide and suicide-attempt prevention

The predominant number of suicide attempts fails – with severe consequences. Also, suicide is still a taboo. DIGNITAS engages in education, clarification of facts and prophylaxis of suicide-attempts.

Patient’s Instructions

Also called Advance Directive, Advance Decisions to refuse treatment or Living Will. For the duration of their membership, DIGNITAS supplies its members with legally effective Patient’s Instructions basing on Swiss law. These must be respected by doctors and hospital nursing staff.

Accompanied suicide

In case of an illness which will lead inevitably to death, unendurable pain or an unendurable disability, DIGNITAS can arrange, on reasoned request and medical proof, for its members the possibility of an accompanied suicide.

Membership

In principle, all adults can become ordinary members of DIGNITAS even if they are not resident in Switzerland and have foreign citizenship. However, we must make it quite clear that DIGNITAS can only provide its members with support within Swiss national territory.

Members who want to secure the services of DIGNITAS have the right to legally effective Patient’s Instructions, to being accompanied at the end of their lives and also to assistance to an accompanied suicide. In addition they are entitled to counselling, so far as the association can provide it, in everything concerning their human dignity in life and in death. DIGNITAS attaches great importance to providing its members with contacts for whom humanity is of great value. The DIGNITAS motto “To live with dignity – To die with dignity” is a promise.

In order to join DIGNITAS, one only needs to fill in the declaration of membership and send it to DIGNITAS. DIGNITAS will confirm the acceptance of membership in writing and provide the DIGNITAS Patient’s Instructions form. With this letter the new member also receives an invoice with detailed payment instructions. The member fills in the Patient’s Instructions and returns the original to DIGNITAS. DIGNITAS registers the Patient’s Instructions and provides the member with sufficient copies. This concludes the process of becoming a member of DIGNITAS.

Declaration of membership

First, please read the information-brochure carefully. The declaration of membership is binding and you are obliged to pay the one-off joining fee of Swiss Francs 200 and the subscription of Swiss Francs 80. Upon receipt of your declaration, we shall send you a confirmation of your membership as well as an invoice with payment instructions. I hereby enrol as a member of "DIGNITAS - To live with dignity - To die with dignity" and I have read and understood the DIGNITAS information brochure.

I am aware of the conditions:

One-off joining fee Swiss Francs (CHF) 200 (approx GBP £ 164 / USD $ 211 / Euro € 193); annual membership subscription at each individual member's discretion, yet, at least CHF 80 (approx GBP £ 66 / USD $ 85 / Euro € 77). People living in very modest financial circumstances may apply for a reduction or even exemption of these fees. The reduction of or exemption from contributions is discussed and agreed beforehand.
