最近刚给爸妈办了绿卡, 我们在准备材料的时候都付上一个cover sheet, 注明了package里面都准备了什么材料.  一来让移民官看了一目了然, 二来自己准备起来也非常organize, 最后寄出前检查材料的时候也很容易check. 

在这里贴一下我们当时用的cover sheet, 希望对需要给父母申请绿卡的筒子们有帮助.   我贴的是妈妈的版本, 爸爸的基本一样哈.

Petitioner:     [Son/Daughter’s name]

                        (Child, US Citizen, D.O.B.: Month, day, year) 

Beneficiary:  Mother’s Name

           (Mother, B1/B2 visa, Citizen of P.R. China, D.O.B.: Month, Day, Year)

Purpose:  Application to adjust status of [Mother’s name] (currently in US with B1/B2 visa) to permanent resident status based on a relative petition by his/her [daughter/son, Name], a US citizen.

1.      Checks:

         a.       A Check of $1,070 (including $985 for I-485 application fee plus $85 for biometrics fee)

         b.      A Check of $420 (for I-130 application fee)

2.      Photos: two passport-style color photos of [Mother’s name]

4.      Form I-130, Petition for Alien Relative

8.      The following supporting documents from [son/daughter’s Name], the Petitioner (for I-130 & I-864):

         a.      Evidence of US Citizenship of [son/daughter’s Name], – Copy of USCIS Naturalization Certificate

         b.      Copy of notarized birth certificate of [son/daughter’s Name], showing names of both parents

         c.      Copy of notarized parents’ marriage certificate

         d.      Copy of employment verification letter from [Company’ name ]confirming [son/daughter’s Name] full time employment status with annual salary of [####]

         e.      Copy of Federal Income Tax Returns from the most recent 3 tax years (2011, 2010 and 2009), including copies of W-2s and Form 1099s

         f.       Copy of most recent 6 months’ pay stubs of [son/daughter’s Name], from employer [company name]

9.      The following supporting documents from [mother's Name], (for Form I-485):

         a.       Copy of notarized birth certificate of [mother's Name],

         b.      Copy of Passport Pages with US Visa and admission stamp

                               i.      Copy of the passport holder’s information page

                               ii.      Copy of US Visa (B1/B2) page with US immigration admission stamp

                              iii.      Copy of I-94 with US immigration admission stamp

         c.       Copy of notarized certificate of no criminal record during [parent’s Name]’s residence in China
